This application concerns only the AZ Holistic Sport Clinic customers.

Access to this application have exclusively the customers of the clinic, only after they have signed the Data Confidentiality Agreement and GDPR in the clinic.
The login information are given by the Receptionist or the Responsible Nutritionist.

Every customer has access only to his/hers personal data.
Every customer has the right to view, delete and edit his/hers information.
In order to do so, the customer must ask the clinic's person in charge and relevant instructions will be given.
